Who We Are

InterMed IPA CTO is a UNIQUE Care Transformation Organization (CTO) formed by a group of independent primary care physicians who understand firsthand the obstacles that clinicians face every day in providing comprehensive quality care to their patients.

Our interdisciplinary support team has years of experience working in a diverse range of clinical environments, and understands that each practice is unique in its individual strengths and needs. Our members have served on the boards of various ACO's and IPA's, and all our member practices are PCMH Plus designated.

What makes us different is the fact that, for the first time, physicians have come together and formed their own Care Transformation Organization, because we all know no one else understands patient care better than we do.

Our Mission

At InterMed IPA CTO, our mission is to leverage our experiences in providing holistic longitudal and episodic patient-centered care management, and to support each practice's care delivery transformation.
